You could add a bail and run a chain [or leather, but I'll just say "chain" for ease of writing] through the heart.
You could run a chain through each side of the heart.
You could wire wrap through the heart and attach it to a chain.
You could put a headpin through the middle and attach.....
You could make several and put them on the chain in stations.
